Aging Resource Navigator

Job Title: ElderCircle Community Aging Resource Navigator
FLSA Designation: Hourly
Hours: 32 Hours per week; M-F Between the hours of 8:00am-4:30pm; may include occasional evenings or weekends for training/events upon request
Reports to: Executive Director and Itasca & Northern St. Louis Counties Aging Services Resource Network

Job Summary
ElderCircle is seeking a personable, self-motivated, reliable, and highly-organized Community Aging Resource Navigator who possesses a passion for providing resources to community agencies that serve older adults and their caregivers. This person will be responsible for the development, organization, quality assurance processes, and the implementation of the Aging Resource Network projects. The ideal candidate is capable of managing their own workload, prioritizing tasks, and thinking ahead to meet the needs of a growing organization.

Vision Statement: The Itasca and Northern St. Louis County Aging Services Resource Network is a collaboration of organizations coming together to promote safe, active, and vibrant lives to older adults and their caregivers. Service options, quality care, community engagement, and inclusivity are the pillars of this healthy aging network – designed with guidance from older adults in the area.

Responsibilities and Duties
Daily Activities:

  • Represent ElderCircle and the Itasca & Northern St. Louis Counties Aging Services Resource Network internally and externally in a professional, courteous, and helpful manner.
  • Build relationships with aging service organizations and enroll them in the Resource Network, utilizing the Memorandum of Agreement and universal referral form.
  • Provide and enhance supports to families, friends, and community members of older adults by increasing awareness and knowledge of community resources and related events.Ensure that all activities are conducted in compliance with all applicable Federal and State laws, rules and regulations, HIPPA, including Civil Rights.
  • Develop compelling and accurate copy for letters, emails, memos, reports, and proposals for stakeholders and the community on behalf of ElderCircle and the Resource Network
  • Assist with the design and production and distribution of programs, flyers, and other marketing materials upon request.
  • Work with Aging Resource Network to develop; and implement a community outreach plan.
  • Work to identify new community partners as well as maintaining relationships with current partners.
  • Track all referrals used through the universal referral process.
  • Provide quarterly referral data to participating agencies.
  • Provide quarterly check-in with participating agencies
  • Anticipate, identify, and document protocols, processes, and procedures. Make recommendations for improvement as appropriate.
  • Work on creating tracking programs.
  • Assist in the development of any programs which may include web-based programs.
  • Uphold a strict level of confidentiality
  • Other duties as assigned

Perform Community Outreach and Public Relations Activities:

  • Work on updating and verifying current information that should be included in the Aging Resource Guide, and add additional information to the resource guide.
  • Assist in preparation and execution of all outreach activities.
  • Assist in social media communications.
  • Prepare communications and marketing materials (including print materials, flyers, press releases, etc.)
  • Provide project/event coordination support by helping internal and external parties organize various components needed to initiate, run and conclude projects.
  • Other duties as assigned

Qualifications and Skills
Essential Qualifications:

  • Representational and public speaking skills
  • Excellent communication skills, both verbal and written, including strong editing skills
  • Exemplary planning and time management skills
  • Demonstrated ability to manage multiple tasks, meet deadlines, and stay organized while providing an acute attention to detail
  • Willingness to be trained in ElderCircle’s electronic tracking system
  • Social Media experience in marketing

Preferred Experience, but Not Required

  • Managerial or supervisory experience
  • Experience working with community leaders of diverse backgrounds
  • Experience providing great customer service on the phone and in-person

Preferred Knowledge, Skills, and Abilities

  • Advanced ability to use Microsoft Office and Excel
  • Ability to work in a small open office setting unsupervised and with regular interruptions
  • Ability to learn multiple computer programs specific to the program

Physical Requirements

  • Hear and speak to use the telephone and communication with people
  • Frequent travel throughout Itasca and Northern St. Louis County
